Accessing medical support
In hours: 8am – 6.30pm, Monday to Friday – medical support for care home residents is provided by local GP practices.
Each care home has a named clinical lead aligned to it from a local Primary Care Network that supports the establishment of multidisciplinary teams and weekly home rounds. Speak to your care home manager for more information about your designated lead or contact BBCCG.firstname.lastname@example.org
Out of hours: 6:30pm- 8am and weekends, dedicated NHS 111 line for care homes
All care home staff can access a dedicated NHS 111 service is in place to provide Care Home staff with rapid access to GP support out of hours.
How can I access this?
Dial 111 when given an option to do so dial *6
What will happen when I call?
Your call will be answered by an NHS 111 Service Advisor as soon an advisor becomes available. By dialling *6 the advisor will be alerted that the call they are about to take is from a care home.
The service advisor will take the patient’s details, complete a short screening process and pass the caller to a clinician within the Clinical Assessment Service (CAS) for a detailed assessment by telephone.
Following the assessment, the clinician will identify the appropriate course of action that should be taken in terms of continued care for the patient.

Urgent care for help in managing a deteriorating resident or client
8am – 10pm, 7 days a week, and up to 4 times a day

Urgent Community Response Team offers a consistent approach to managing acute episodes of ill health in the community with prompt specialist interventions. UCRT services will offer short-term, intensive, time-limited support of up to 14 days for a physical health crisis or 3 days for a social crisis. The UCRT will operate in the most appropriate setting (including care homes), stabilising the presenting crisis and reducing avoidable attendance or admission to hospital.
The service provides an integrated, urgent, reactive response to patients in their usual place of residence based on assessed clinical or social need. The UCRT will contact the referred person within 2 hours.
